Help needed in translation of Document in Laotian

Hi guys, wonder if anyone can help me with this translation. Just want to know if it stated the entry and exit point in Laos cause I might be changing my route.

Used some apps to scan and translate but can only figure out the name, chassis number, license plate number, from which day to which day. Hope you all can help. Thanks

It's notification from the Lao Embassy in Singapore to Immigration officials of details of your upcoming trip to Laos, (if you are Tan Joo Peng???)

Currently you are scheduled to enter and leave via Chiang Khong - Huey Sai, 20th December until 28th December 2014.

It states both the entry and exit points, but they are the same: Chiang Khong - Huay Sai, Bokaew Province.

Why do you need this document? Are there special circumstances? I've just taken my bike to Laos without any need for an official notification letter.
